When to Use the Plan of Study Form

This form must be completed with an Academic Development and Recovery Advisor and your assigned major advisor, and then submitted with your Reinstatement Appeal Form. You will need this form if you are appealing for one of the following:

  • immediate reinstatement
  • early reinstatement
  • requesting to return after a third dismissal

How to Submit a Reinstatement Appeal Form and Plan of Study

  1. Schedule a 60 minute appointment  with an Academic Development and Recovery (ADR) Advisor on the Academic Development and Recovery webpage or by calling (208) 426-4049.
  2. Schedule a meeting with your assigned major-specific advisor. Their contact information can be found in your Student Center.
  3. Complete and sign the Plan of Study form following your discussions with an ADR advisor and your major-specific advisor.
  4. Attach the Plan of Study form to your Reinstatement Appeal Form as supporting documentation.
  5. Email the forms to registrar-appeals@boisestate.edu.

What is the difference between the Reinstatement Appeal Form and Plan of Study form?

The Plan of Study is supporting documentation for your Reinstatement Appeal. You should submit a Plan of Study with your Reinstatement Appeal Form.
