It is the responsibility of the student to review and understand the confidentiality and privacy of their records. If you choose to allow information to be released to an individual outside the university, you must complete and sign a Release of Information Form and submit to the Registrar’s Office. Before signing, review the Confidentiality and Privacy page carefully.
The form remains on file indefinitely. If you choose to withdraw your release at a later time, you must complete the rescind portion of the form and submit it to the Registrar’s Office.
