PACS Accommodated Exam Form

Student and Instructor Information

Faculty may request accommodated exams through PACS for students with approved EAC accommodations. Please submit one form per exam in the course.

List the students who need to take this exam. Provide the student's first and last names and their Boise State email address.

We only receive requests sent from Boise State email addresses (@boisestate.edu). Please use your Boise State employee or instructor email address to help us process this request for you.

Exam windows must be at least one day. Please exercise flexibility when choosing exam date ranges as seating is limited.

List permissible test aids or upload a test aid for student use during the exam.

List any test aids allowed on this exam.

Upload a test aid document if appropriate.
