Program Proposal Information

Program Delivery (Please check one):(Required)

Lecture (One, two-hour presentation. Allows for a 10-minute break and approx. 10-15 minutes for questions. Total time: 90-100 minutes.)

Short Course (Same as Lecture, but meets for two or three sessions on the same day of the week and same time of day.)

Workshop or Seminar (Intended for smaller audiences, discussion, and/or hands-on activities. Please indicate number of sessions (one, two, three, or four) in Proposal Description.

Special Event: Guided tour of a facility or exhibit. These are usually a minimum of one hour to a maximum of two hours in length. There is no honorarium for these events, but the Osher Institute may make a contribution to the organization.

Special Event: Musical or theatrical performance with lecture/talk with the audience. The maximum time is two hours.

Special Event: Meet the author. Usually one hour in length with time for Q&A from the audience.

I am NOT an employee of Boise State University. I would accept the honorarium via Independent Contract. This requires an agreement to onboard with Boise State’s third-party vendor payment system, Payment Works. If I fail to complete the documentation by the deadline, I understand that this will forfeit an honorarium and I will instead receive a one-year membership to the Osher Institute.

I am currently a full-time, benefits-eligible faculty member or employee of Boise State University. I understand that it will be payable via University payroll.
