Request a Consultation

To request a visit from our program, you will need the following information:

  • Company legal name
  • Street address
  • Mailing address
  • Contact name, phone number(s), and email
  • Number of employees companywide (including any other locations)
  • Number of employees at your location
  • OSHA enforcement activity in the past 12 months

If you have an open case with OSHA enforcement, we can’t come onsite until the case is closed. You can still make a request.

  • Type of services you would like.

The basic service is a full service visit. It includes a walkthrough and a review of your written programs. You can also request a limited service visit. This type of visit can be just a walkthrough, or just a written program review.

  • Finally, we’d like to know how you found out about our program.

  • (For example: Long distance trucking company with a cold storage facility. We also re-package previously frozen items.)
  • (For example: We would like help with machine guarding on our production lines.)
  • (A full service request consists of a walkthrough of your facility to identify hazards, plus a review of written programs. It is the basic consultation request.)
  • (For example: We need air monitoring in our spray booth.)
