Hello, my name is Becky Clawson and I’m the application team manager for the advanced medical imaging program. Welcome to the program application tutorial for how to apply to your modality year. During this tutorial, I will break down the steps for how to apply and go over the application with you. Let’s start by answering some common questions.
Who needs to apply to their modality year?
All students must apply to their modality year using this new application process. This application is required for every student who has not yet started modality courses, regardless of any past application materials submitted.
When do I submit my application?
Applications for the AMI modality year are due no later than March 1 for a fall start. This deadline applies to all modalities.
For example, if your modality courses are scheduled to start in fall of 2025, your program application materials are due no later than March 1, 2025. Do not wait until the deadline to apply, however.
If you live outside the Treasure Valley, Idaho area, you will turn in your clinical site survey with your application packet. The sooner we receive this information, the sooner we can begin working on your clinical affiliation agreement with your facility.
How do I complete the application?
Your three-step application process is outlined in the AMI Student Resource site located within Canvas. This outline will prepare you for the application, tell you how to complete the application, and tell you what you need to finalize the application process. As I said, the AMI Student Resource site is located in Canvas within your myBoiseState account.
Click on Canvas. On your menu on the dashboard of your Canvas account click on the advanced medical imaging plan site and then navigate to module 12 to view this three-step process. You’ll click on module 12.01 How to apply to your AMI modality year. This first section walks you through what to prepare for before you begin the application form.
You’ll want to read through this list [before you begin your application, have the following materials ready], and then I’ll give you more details for how to complete these materials. You’ll need your resume, any health care credentials, the names and contact information for three references (one academic, one from previous employment and one personal character reference). Any new transcripts since you’ve applied to Boise State. A letter of application (you may use your statement of purpose if you’d like from your AMI 234 Intro to Clinical Experience course). You’ll also need a debit or credit card to pay the $20 application fee. You can read more about these materials at this link here [click the next button to go to module 12.02]. The application information page. If you click on this [Application Information page], you’ll want to read this thoroughly from top to bottom. If you scroll down, you can read each step and a little paragraph to begin with.
If you keep scrolling, you’ll read more details. However, I do want to note here that you will be asked several times to submit your transcripts, and you do not need to submit transcripts again. Please note you only need to submit any new transcripts since applying to Boise State. As you scroll down, you’ll read in more detail about each application step.
The letter of application is outlined here [on the Application Information page] in case you need to rewrite that letter if you do not have a statement of purpose prepared already. As you keep scrolling, I will highlight this section here for references. You are required to choose three references to submit reference forms on your behalf. You can download the reference form here [in the Instructions for references section on the Application Information page] as a PDF, and this reference form is one you will need to give to your individual recommenders so that they can submit this on your behalf.
This is the form that you will be giving each recommender. There is information here [on the reference form PDF] about where they need to either email or mail this form in, and these must be received no later than March 1 to complete your program application. If you scroll further [on the Application Information page], you can look at the reference categories in greater detail. Your academic reference work related reference and general character reference information is all here, so make sure to read through this and choose your recommenders wisely.
Begin your application
Once you’ve prepared all of that information, you can begin the application by clicking the Department of Radiologic Sciences application link here [the link is in module 12.02 in Canvas]. I’ve started this application for us and we’ll walk us through page by page. Here, Buster Bronco has started the application. You’ll want to fill out all the fields of the application that are marked as required. Choose your modality and type of degree or certificate you’re applying for.
You could list in this box [“If applying to multiple programs, please list the program(s) you are applying to in the order of your preference” on the application] a second choice if you have another modality you want to select as a backup. This question here is one that asks you if you are completing clinical rotations within the Treasure Valley or outside the Treasure Valley. This is defaulted to yes for someone who is going to be completing clinicals in the Boise area.
If you’re outside of the Treasure Valley, you’ll want to switch this to no so that this bottom part expands and you could enter in the city and state that you will be completing your clinical rotation in. As you scroll down, there’s more information to enter. And when you’ve uploaded all of that, you’ll click next and go to page two.
As you can see, if you do not fill out all of the fields, you will not be able to click to the next section of the application, so make sure to fill in all required fields. Here we are on step two of the application form. You’ll enter in your work history or volunteer history in the sections you can use your resume to have in front of you to refer to for this section. Here again, you’re being asked to submit transcripts, but just go ahead and click that you’re acknowledging transcripts are needed, but for you, it’s only if you have new transcripts to submit to the university since you were admitted. This is a nuanced part of the application, but I wanted to call out again, you do not need to submit all of your transcripts, just the new ones.
Submit references
Clicking next to page three of the application. This is where you’ll submit your references. There’s the reference form linked again. You’ll enter just the first and last name of each reference and then click next. Page four of the application walks you through some awareness and consent pieces, so you’ll read through and click I agree if you do agree to each section. And at the end of this page you can scroll way down and click that you have completed the application thoroughly and click submit.
Pay the application fee
You will then be asked to pay your application fee of $20 online. You can submit the form without paying the fee. However, you will need to pay the application fee before March 1st to complete your application. Once we’ve received your application form, you will receive an automated email letting you know that we’ve received it.
Submit the clinical site survey
Moving to the final stage of the application process [in Canvas click next to go to module 12.03], this is all about finalizing your application. If you live outside the Treasure Valley area of Idaho, you will be asked to submit two additional things to complete your application. One is the clinical site survey that we referred to in the beginning. The survey form must be filled out by the department manager at your clinical site. You can download the appropriate clinical site survey form in module 10.02 of this site.
In order to show you this, I’ve already pulled this up in module 10.02 complete clinical site survey is the title and if you scroll down you can see each clinical site survey form linked by modality. Choose the appropriate form for you and send that site survey to the department manager who has given you a verbal agreement for your clinical site.
They will need to complete that form, send it back to you and you will need to email that to me, Becky Clawson. Some of you may already have submitted this and it went to your program modality program director, and that is okay as well. But I will need a copy emailed to me to complete your application. Virtual interview recording
You’ll also need to complete a virtual interview recording. This recording is completed in Panopto. You may also use the video you previously recorded in AMI 234 Intro to Clinical Experience. That would still be in your Panopto account as a Boise State student. If you would like to share that with my email address, that would be acceptable as well.

So if you click on this, this outlines the virtual interview instructions and questions that you will need to answer. Once you’ve completed these two things, please email both of these to the beckyclawson@boisestate.edu email address to finalize your application.
In-person interview
If you are living local to the Treasure Valley area, you will need to attend an in-person interview in March if you were selected for local clinical placement. This only applies to students living in the Treasure Valley area who are interventional students in the AMI program.
