Forms

Forms for the PAR Reporting process will be delivered to departments via Google Shared Drives. Downloadable versions of the forms are available to download below.

All departments with an undergraduate program going through the PAR process this year will need to submit the Finishing Foundations form. The form can be accessed on the University Foundations website.
