Eligible employees may choose between the three medical plan options shown here, with coverage beginning as of the first of the month following date of hire, or following the date they become eligible, if later. Changes from one plan option to another are only allowed during the annual enrollment period, for coverage beginning July 1.
Note: In all cases, insured should check with Regence Blue Shield at 1 (800) 854-5585 to determine if the treatment or service being considered requires prior authorization. All inpatient admissions and emergency admissions require Inpatient Notification Review or Emergency Admission Notification, as appropriate.
