Supervisor Forms
Student Internship Application for Academic Credit
Once the student has arranged for an internship, the will be asked to submit the “Internship Application for Academic Credit,” which is available online through the Career Services website (the form is under “Internship Application Process”). While completing your application for academic credit, the student will be prompted to designate the department (history), the coordinator (Emily Fritchman-Mahaney), and information specific to the details of the internship, including the description of the work to be completed. The supervisor will be asked to review the application prior to final approval.
Final Supervisor Evaluation
Final Supervisor Evaluation: Prior to the end of the semester the internship coordinator will request a written evaluation from you, the supervisor. The student will be provided with a copy of the evaluation by the coordinator prior to turning their grade into the registrar. A copy of this supervisor form is attached below.
