Under Boise State University Policy 12060 (Protection of Minors), non-University programs that accept custody and control of the minor participants must ensure their employees, volunteers, and representatives undergo a criminal history background check, minor abuse prevention training, and agree to have a standard of conduct in place designed to prevent the abuse of minors. Please review the following policies and complete the following attestations:

University Policy 7005 (Background Verifications)
I certify I understand that as a non-University program, all employees, volunteers, and representatives working with the minor participants must have completed a criminal history background check that is at least as stringent as those conducted by the University under Boise State University Policy 7005 (Background Verifications).
I certify I understand that that as a non-University program, all employees, volunteers, and representatives working with the minor participants must have completed a minor abuse prevention training that meets or exceeds the minimum requirements outlined in Section 7.4 in Boise State University Policy 12060 (Protection of Minors).
I certify I understand that that as a non-University program, I must have standards of conduct in place designed to prevent the abuse of minors outlined in Section 8.e. in Boise State University Policy 12060 (Protection of Minors).
I certify I understand that that as a non-University program, the University may request documentation necessary to verify that I have met the above requirements for up to one year following the last day of the event. I understand if I fail to provide any requested documentation necessary to verify this compliance, future programs may be canceled or not allowed to use Boise State University property and facilities.
I certify I understand the following administrative fee structure to help ensure appropriate staffing for the Minors on Campus program and to maintain compliance with University policies: $5 per registrant for all day-use only programs, regardless of registration fee; $10 per registrant for all overnight programs.
