OIT Account Requests
Getting Started
All University computer accounts require an Account Request Form accessed online, printed and signed by the requestor and supervisor, and delivered to the OIT Help Desk.
Before You Begin
- Ensure that all new hire paperwork (including an I-9 form) has been completed and processed by the Office of Human Resource Services.
- You will need the account requestor's employee ID #, BroncoWeb username, and specific information about the account(s) being requested.
- If the requestor is an affiliate (not paid by BSU), download and complete the Affiliate Status Form, then contact the Office of Human Resource Services to obtain an employee ID #.
Account Request Form Instructions
Step 1: Browse to the Account Request Form at http://oit.boisestate.edu/accountrequest/. (The form will open in a new window.)
Step 2: The first few pages of the Account Request Form will ask for general information about who the account(s) will be for, the type of request you are processing (new hire, change existing account, etc.), and the account user's information. Please complete all fields and review them for accuracy.
Step 3: The next part of the form will ask you to choose the type of account(s) you are requesting or modifying. Whenever possible, we recommend including an equivalent employee. To help you understand what type of accounts can be requested, please review the following information; you may also want to review the Account Request Frequently Asked Questions page for account requests.
If you are processing a Delete request, please make sure the employee information for name, username, and employee ID are correct and we will take care of removing all access for that account.
Special Note for GroupWise Accounts
Note that any GroupWise account created for a student will act as the student's default e-mail account, and e-mail sent from the student's BroncoMail account will appear as sent from GroupWise. If the e-mail is replied to, it will go to the student's GroupWise account. If a message is intended for delivery to the student's BroncoMail account, the sender must specify the BroncoMail address (e.g., username@u.boisestate.edu) instead of using the 'reply' function.
Step 4: Review and print your request. (Note: This is not an electronic form!)
Step 5: Obtain the appropriate signatures, then fax your completed request to the Help Desk at 426-4232.
